- The information collected will be used for providing treatment to your child/children. Personal information will be used for the purpose of addressing correspondence to you, as well as processing payments.
- We may disclose health information to other professionals involved in your child’s care, or request it from them, if it is necessary in the context of your child’s treatment. This may include someone like your child’s GP, Paediatrician, Psychologist, Occupational Therapist or educator. Any disclosure would be limited to relevant information only.
- Your assistance is requested in providing us with updated personal and health information at subsequent visits (change of address, an additional or new service provider, such as Paediatrician, Occupational Therapist etc.)
- Your information, treatment records and any other material relevant to your treatment will be retained in a secure manner. When no longer required, information may be destroyed in accordance with Government regulations.
- We will maintain and abide by a Practice Privacy policy that conforms to Government regulations. You may request a copy of this privacy policy at any time.
- You may request copies of records or reports at any time, or seek an explanation. Statutory fees will apply in relation to the type of access you seek.
Your personal and health information will be treated with the utmost confidentiality. Disclosure of such information will not be made to any person not involved in either your child’s treatment (or in the administration of this practice), without your prior written consent.
